Regarding insurance coverage, please note: If you have health insurance, (or Auto or worker's compensation insurance, depending on the circumstances) we will verify your chiropractic coverage, usually by your second visit.

1.                  If You have Insurance

·              Please contact your insurance and know your coverage type- HMO If you have an HMO, you will need a referral for specialist care including Chiropractic. It’s always good to know your policy before hand.

·              When you arrive, you will also be asked to present a copy of your driver's license or legal identification and current valid insurance card as proof of insurance.

·              Our billing staff is available to answer any questions regarding your bill or insurance payments.

·              If your insurance carrier has changed since your last visit, please notify the front desk personnel in order to prevent delay in processing your claims. We will also require a referral to a specialist, pre-certifications or authorizations if necessary.

·              We will file a claim with your insurance for your visits. Your monthly patient statement will show all amounts due. If you have a patient balance due on one or more accounts and insurance pending on other accounts, your statement will reflect both patient and insurance pending amounts. To lower administrative costs for your care, we require prompt payment of all amounts you are personally responsible for.

2.                 If you do not have insurance/chiropractic coverage:

·        We offer convenient payment plans for all cash or self pay patients.

·        "Care Credit" plans are available in the clinic for financing your care program. Our patient coordinator will be more than happy to answer any financial questions for you at the time of your visit
